The setting is a parallel world to Fate/stay night where the Greater Grail mysteriously disappeared from Fuyuki after the Third Holy Grail War. After many years of silence, around the same time as the Fifth Holy Grail War would have happened, the Yggdmillennia, a family of magi, openly declares their secession from the Mage's Association, and that they are in possession of the Grail. The Association dispatches fifty magi to retrieve it, and all but one are instantly slaughtered by a mysterious Servant. The one remaining manages to activate the reserve system of the Greater Grail, allowing for the summoning of fourteen Servants in total. In the city of Trifas, two factions will fight for the control of the sacred relic, each of them possessing their own team of seven Servants : the Black Faction whose members are part of Yggdmillennia, protecting the Grail, and the Red Faction whose members were sent by the Mage's Association, trying to take the Grail back. For an event of this scale, the Grail itself summons its own Servant, the holy Ruler, to oversee the conflict. This marks the start of the Great Holy Grail War.

Inaba Yuushi's parents died in his first year of middle school, and he moved in with his relatives. Though they did care for him, he could tell he was a burden. After he graduated, he happily prepared to move to a high school with a dormitory. Unfortunately, the dormitory burned to the ground before he could move in! The kingdom of Dowa, which is subdivided into 13 states, is celebrating its monarch's 99th birthday. These 13 states have many agencies that are controlled by the giant organization known as ACCA. Within ACCA, Jean Otis is the second-in-command of the inspection agency. His agency has ten people placed in each of the 13 states, with a central office in the capital city. They keep track of all the activities of ACCA across the kingdom, and keep data on each state's ACCA office flowing toward the central office. Jean also often has business trips from the capital to the other districts to check on the situation and personnel there.

This man, Murasaki, sends Toyohisa into the nearest door where he wakes up in another world. There, Toyohisa meets other great warriors like him who have been transported as well, to be part of a group known as "Drifters". This world contains both native humans and a number of fantastical races, including elves, dwarves, and hobbits. However, the world is at war, with the humans waging a losing war against another group of great warriors, the "Ends", who wish to take over the world and kill all of the Drifters. Under the Ends' command are many terrible creatures, including dragons, which they use to destroy everything in their wake. At the start of the series, the Ends' army has control of the northern part of the continent, and are currently trying to invade the south through a pivotal fortress at the northernmost tip of a nation called Carneades. Meanwhile, the "Octobrist Organization", a group of human magicians native to this world, attempt to bring together the many individual Drifters to save their world from the brutal Ends.

Guided by one particular song, young elementary school student Suzuhara Sorata, from the Heisei Era, is warped through a time skip to an unfamiliar place that looks much like the towns from the Meiji or Taishou period from his textbooks. In this world, he meets three young ladies and nine young men and joins them on a journey aboard the mysterious Norn ship, a giant globe that floats in midair.

The Blue World, which symbolizes reason, is under constant attack by the creatures called the Nightfly O'Note originating from the Red World, symbolizing instinct. Akira Asagi, the 17-year-old protagonist who is a researcher at Blue World, is nominated to lead the war at Ryuukyuu LAG, a proud defense facility on the islands of Ishigaki and Iriomote. She meets the sixth combat unit "IS," which was formed four years after a fierce battle that annihilated the fifth unit. Akari must lead these six riders, also called the Scared Rider Xechs, to fight against the other world while at the same time deepening the relationship among them.

Ken Kaneki becomes close close with a girl named Rize, who unbeknownst to him is a "Ghoul", a monster that lives off human flesh. He too becomes a ghoul when part of Rize is transplanted to him.

In an attempt to colonize Mars, 21st century scientists were tasked with warming up the planet so that humans could survive on its surface. They came up with an efficient and cost effective plan of sending cockroaches and mold to the surface so that the mold would absorb the sunlight and purify the atmosphere, while feeding the insects whose corpses would help the mold spread throughout the planet. Five hundred years later, the first manned ship to Mars lands on the planet and the six crew members are attacked by giant mutated humanoid cockroaches with incredible physical strength later labeled as "Terraformars". The crew members are easily wiped out, but not before sending a transmission back to Earth. Decades later, a multinational expedition is sent to exterminate the mutated bugs and take control of the red planet only to be wiped out as well, except for two survivors who managed to return to Earth, with one of them swearing to one day return to Mars and avenge their fallen companions. As a third expedition is assembled, questions are raised about the true origin of the Terraformars and their connection with a mysterious disease that is afflicting mankind back on Earth. To fight the Terraformars' strength and agility, the members of the second and third expedition undergo genetic modification to inherit the characteristics of other organisms, only possible after having a special organ implanted inside their bodies with only a 30% chance of surviving the surgery.

The "life-and-death sibling" story follows Utsutsu and Yume Hasegawa, a boy and his little sister who find themselves all alone. One day, Yume sees a mysterious red butterfly and her body undergoes a strange metamorphosis-into a creature that eats humans. Utsutsu struggles to find a way to restore his sister.

As the mystery deepens, Real is led to a world outside the city, a world far beyond the boundaries of Romdeau.
